A Single‐Dose Study of Mexiletine (Kö 1173)

Abstract

Serum concentrations of mexiletine after a single dose were determined in 8 adult Caucasian males with complex partial seizures who were continuing to receive other antiepileptic drugs. Two patients each received a single dose of either 100, 200, 300, or 400 mg mexiletine. Serum concentrations were determined by two gas chromatographic methods. Serum concentrations ranged up to 795 ng/ml. Peak concentrations occurred 1 to 3 hr after administration of the drug and were significantly different between the 100‐ and 300‐mg, 100‐ and 400‐mg, 200‐ and 300‐mg, and 200‐and 400‐mg doses. Differences between the other doses were not significant. Serum concentrations declined monoexponentially. Half‐life ranged from 2.7 to 7.2 hr. Numerous papers have appeared in the European literature on the use of mexiletine to treat cardiac arrhythmias. Preliminary studies in the United States suggest the use of mexiletine as an adjunct for therapy of epilepsy. Les concentrations sériques de mexiletine aprés une dose unique étaient étudiées chez 8 adultes caucasiens males presentant des crises partielles complexes et qui continuaient a recevoir d'autres medicaments antiepileptiques. Deux malades ont re?u chacun 1 dose unique de 100, 200, 300, ou 400 mg du proaun. i^es concentrations seriques étaient determines par 2 methodes de chromatographic en phase gazeuse. Les concentrations seriques se rangeaient au‐dessus de 795 ng/ml. Des pics de concentrations apparaissaient 1 a 3 heures apres 1'administration du produit et étaient diffeVents de facon significative entre 100 et 300 mg, 100 et 400, 200 et 300, 200 et 400 mg. Les differences entre les autres doses n'etaient pas significatives. Les concentrations seriques de‐clinaient de fa?on monoexponentielle. Le temps de demi‐vie s'etendait de 2.7 a 7.2 H. De nombreux articles ont apparu dans la literature europeene quand a 1'usage de la mexiletine dans le traitement des arythmies cardiaques. Des etudes preliminaires aux U.S.A. sugge'rent l'utilisation de la mexiletine comme adjuvant dans le traitement de l'Spilepsie. Se han determinado las concentraciones sSricas de mexiletine administrado en dosis unica a 8 adultos varones con ataques parciales complejos que continuaron su regimen anticon‐vulsivo habitual. Cada dos enfermos recibieron una sola dosis de 100, 200, 300 o 400 mg de mexiletine. Las concentraciones en suero se determinaron mediante dos metodos de croma‐tograf fa de gases y alcanzaron niveles hasta 795 ng/ml y los más altos ocurrieron entre 1 y 3 horas despues de la administration de la droga. Fueron significativamente diferentes entre las dosis 100 a 300, 100 a 400, 200 a 300 y 200 a 400 miligramos. Las diferencias entre las otras dosis no fueron significativas. Las concentraciones sericas declinaron monoexponencial‐mente y su vida media oscilo entre 2,7 y 7,2 horas. En la literatura europea han aparecido muchas comunicaciones sobre la utilization del mexiletine en el tratamiento de las aritmias cardiacas. Estudios preliminares en Estados Unidos sugieren el uso de esta droga como tratamiento coadyuvante de la epilepsfa. Nach Einzeldosen wurden bei 8 erwachsenen mannlichen Probanden weisser Rasse mit nicht generalisierten Anfällen die Serumkonzentra‐tionen untersucht. Die Probanden wurden weiterhin mit anderen antikonvulsiven Medikamenten behandelt. Jeweils 2 Patienten erhielten einmal 100, 200, 300 oder 400 mg Mexiletine. Die Serumkonzentrationen wurden mit Hilfe von gaschromatographischen Methoden bestimmt. Die Serumkonzentrationen erreichen bis zu 795 mg/ml. Konzentra‐tionsgipfel traten l‐3h nach Applikation der Substanz auf und zeigten signifikante Unter‐schiede zwischen den Dosen von 100 und 300, 100 und 400, 200 und 300, und 200 zu 400 mg. Bei den anderen Korrelationen waren die Unterschiede nicht signifikant. Die Serumkonzentrationen nahmen monoexponential ab. Die Halbwertszeit schwankte zwischen 2.7 und 7.2 h. In der europadischen Literatur sind zahlreiche Arbeiten erschienen iiber die Anwen‐dung von Mexiletine bei der Behandlung von Herzrhythmusstorungen. Vorlaufige Unter‐suchungen in den USA lassen an den Gebrauch als Adjuvans in der Epilepsiebehandlung denken.
