Pharmacokinetic Profile of Flunarizine After Single and Multiple Dosing in Epileptic Patients Receiving Comedication

Abstract

Summary: This preliminary clinical study describes the pharmacokinetic characteristics of flunarizine (FLN) following single and multiple dosing in epileptic patients receiving comedication. Three groups [phenytoin (PHT) only, carbamazepine (CBZ) only, and PHT plus CBZ] of four patients each were studied. Large interindividual differences, but no statistically significant differences in pharmacokinetic parameters, were observed between the three groups. Following a single dose, mean values (and ranges) for apparent clearance, volume distribution, and elimination half‐life (t½) were 0.504 L/h/kg (0.086‐1.119), 12,431 L (1,959‐20,920), and 308 h (61–506), respectively. FLN had no effect on PHT or CBZ steady‐state levels but PHT or CBZ appeared to induce the metabolic disposition of FLN. The effect of dose on FLN kinetics could not be evaluated in this preliminary study. Este estudio clínico preliminar describe las características far‐macocinéticas de la flunaricina después de dosis única o múltiple en enfermos epilépticos recibiendo co‐medicaciones. Se han es‐tudiado tres grupos de cuatro pacientes cada uno (fenitoina só1o, carbamazepina só1o y fenitoina más carbamazepina). Se obser‐varon ámplias diferencias interindividuales en los párametros farmacológicos entre los tres grupos pero sin significatión esta‐di'stica. Tras la administración de dosis única, se obtuvieron los siguientes datos con respecto a los valores medios (y rangos) de los aclaramientos aparentes, la distribución de volumen y la eliminación de la vida media: 0.504 L/h/Kg (0.086‐1.119), 12,431 L (1,959‐20,920) y 308 h (61–506), respectivamente. La flunaricina no mostró ningún efecto sobre los niveles del estado estable de la fenitoina o la carbámazapina pero ambas drogas parecen haber inducido la disponibilidad metabólica de la flunaricina. No fue posible valorar los efectos de la dosis sobre la cinética de la flunaricina en este estudio preliminar. Diese vorläufige medizinische Studie beschreibt die pharma‐kologischen Charakteristika von Flunarizin nach Einzel‐ oder Mehrfachgabe bei epileptischen Patienten unter antiepileptischer Komedikation. Drei Gruppen wurden untersucht: Phenytoin, Carbamazepin, Phenytoin + Carbamazepin. Bei alien drei Gruppen wurden große individuelle Unterschiede, aber kein statis‐tisch signifikanter Unterschied gefunden bezügl. der pharmakol‐ogischen Parameter. Nach Einmalgabe lagen die Mittelwerte für Clearance, Volumenverteilung und Eliminationshalbwertzeit bei 0.504 1/h/kg (0.086‐1.119), 12,431 1 (1,959‐20,920) und 308 h (61–506). Flunarizin hatte keinen Einfluß auf die Phenytoin‐ oder Carbamazepin‐steady‐state‐Spiegel. Dagegen schien sowohl Carbamazepin als auch Phenytoin den Metabolismus von Flunarizin zu induzieren. In der vorliegenden Studie konnte die Dosis‐Wirkung‐Beziehung von Flunarizin nicht geklärt werden.
